I have designed this medical journal to help people with complex medical issues to keep track of symptoms, medication and appointments. There are a range of different pages to record important and helpful information regarding medical conditions.
Pages included are: address book – to keep contact information for specialists you see: food & exercise diary to track eating habits, calorie and water intake, etc.: medication record to record which medications you have taken each day: weekly calendar to record symptoms and activities each day or for use as a weekly calendar: month at a view to keep track of appointments or as a quick view of general health (you could colour code or use smiley, sad faces): thankful journal to help focus on positives and look ahead: blank pages, some are divided into 2 or 3 sections to record any details or to be personalised as you like. If you have lots of symptoms these pages might be better for recording your daily health levels as you will have more space.
You can have 5 main sections in the journal and each of these can be subdivided into smaller sections using the note page dividers.
Making the journal does require quite a bit of cutting, although it is mostly straight lines so a trimmer can be used. Brads or split pins are used to hold the pages in place and allow you to replace them as needed.
This journal is provided for free and can be made for yourself, for a family member or friend, and you are allowed to sell at craft fairs etc.
